Now Here’s my gym membership form that I have modified as circumstances have arose. you can download them for free and open them into an editable file and change the name and address and phone number and a few other things to make it your own waiver then have a printer make them into duplicates. But First, I’d make the front page stuff then put all the legal stuff on the back page then have duplicate copies made in packs of 250

You may copy mine exactly except the logos. Be sure to have an attorney check it over especially if your in another state where laws may be different.
